More than 500 mothers and babies suffered potentially avoidable harm or died at a “toxic” hospital trust, a damning report has revealed.
The biggest maternity review in NHS history has exposed 162 deaths – including 156 children and six mothers – could have been avoided if they were given better care.
The Ockendon report, an independent inquiry into maternity services in Nottingham, has included the cases of 2,500 families over a 20-year period, and serious and deadly incidents in maternity care and found a string of “repeated failures”.
Some mothers left the hospital in terror at the idea of ever having to give birth again.
One told the review about her experience in 2014, “When I discovered I was pregnant with my second child, my immediate reaction was to terminate the pregnancy because I was too frightened to go through the experience I did with my daughter…Having been desperate to have any children this speaks volumes about the level of trauma…”
This review found that none of these incidents was an isolated incident but instead part of a wider culture that treated women with contempt, summarising it as consistent “breaches of fundamental standards of dignity, respect, and professionalism.”
